International Insurance in China: Direct Billing and Pre-authorisation
Check the exact policy, department and planned service. A direct-billing network does not mean every bill, test or treatment will be covered.

International Insurance in China: Direct Billing and Pre-authorisation
Direct billing is a payment arrangement
Direct billing means that an institution and an insurer or claims administrator have an arrangement to settle eligible charges. It is different from paying yourself and later applying for reimbursement. Neither arrangement makes every item in a medical visit covered.
Beijing's official guide says international patients may use direct billing where their insurer has an arrangement with the institution, and directs readers to the institution's insurer list. Do not turn a city-wide institution count into a promise about your own policy. Confirm the named campus and department, not only the hospital group.
Ask both sides before the visit
Give the insurer the institution, service, proposed date and the medical information it requires through its secure channel. Ask whether pre-authorisation or a guarantee of payment is needed. Ask the hospital whether it accepts that authorisation for the exact service. Keep the confirmation reference and the insurer's assistance contact.
| Question | Why it matters |
|---|---|
| Is this policy in the network? | A familiar insurer logo may cover different plans or administrators |
| Is this department included? | Ordinary, special-needs and international services can differ |
| Is the planned service covered? | Screening, outpatient care, admission and medicines can have different rules |
| What remains payable by me? | Deductibles, co-payments, exclusions and limits may apply |
| What happens if the plan changes? | Additional tests or admission may require a new approval |
Insurance decisions belong to the insurer. Clinical decisions belong to the treating clinicians. A coordinator can help you organise the questions but cannot approve a claim or decide which investigation is medically necessary.
Prepare for a payment gap
Ask whether a deposit may be needed while approval is pending and what payment methods are accepted. Obtain itemised bills, receipts and the medical documents the insurer requests. A translation, appointment confirmation or card receipt may not by itself satisfy a claim requirement.
If you must pay first, confirm the reimbursement process and submission deadline directly with your insurer. Do not assume that a declined direct-billing request means the claim is definitely excluded, or that paying cash guarantees reimbursement. Those are separate decisions under the policy.
Questions patients ask
Does travel insurance automatically cover planned treatment?
No. Planned care and unexpected illness can be treated differently. Ask the insurer about the actual purpose of your trip before booking.
Is showing an insurance card enough?
It may identify the policy, but the hospital can still need verification, authorisation and payment of your share. Confirm before the visit where possible.
Can PriumCare guarantee a cashless appointment?
No. Direct billing requires confirmation from the insurer and institution. Any coordination service is separate from insurance coverage.
What if a new test is recommended during the visit?
Ask the clinical team about its purpose and the insurer or billing team about approval and cost. Do not assume it is included in the original authorisation.
