A Visa Guide for Medical Travel to China: Transit, Hainan and Consular Visas
China's published notices do not describe a separate medical-tourism visa. The 240-hour visa-free transit and Hainan's visa-free entry are immigration rules with named limits. A hospital booking is not permission to enter.

Direct answer. A hospital appointment in China is not an entry category. Notices published by the National Immigration Administration do not describe a separate medical-tourism visa for routine outpatient care. The category that applies to you is decided by your passport, your route, your purpose, and the Chinese embassy, consulate or port that deals with you. This page summarises two visa-free arrangements as they stood in the NIA announcement of 20 August 2026, and the point at which you need that authority instead of a coordinator. It does not issue a visa and it does not promise admission.
Key facts, as of the 20 August 2026 notice
| Route | What the notice says | What it does not say |
|---|---|---|
| 240-hour visa-free transit | Nationals of 57 countries, ordinary passport, confirmed onward ticket to a third country or region, entry at one of 65 ports, stay in the permitted areas of 24 provincial-level regions, no more than 240 hours | That a medical appointment is a listed activity, or that a return ticket to the same country qualifies |
| Hainan visa-free entry | Nationals of 61 countries may enter Hainan through its open ports and stay in Hainan for no more than 30 days | That the stay can be used on the mainland outside Hainan, or that a named medicine or device will be available |
| A consular visa | The embassy or consulate that serves you decides the category and the documents | That an appointment letter chooses the category or replaces that office's decision |
| At the hospital | Registration is a separate step and commonly uses your passport | That the hospital can tell you which visa to hold |
The activities named for both visa-free arrangements are tourism, business, visits and family visits. Work, study and news reporting still need the appropriate visa. Medical care is not in that list. Before you buy tickets, ask the airline and the embassy or port whether your purpose is accepted. Do not treat a booking as permission to enter.
The 240-hour visa-free transit
The NIA announcement of 20 August 2026 (Announcement No. 4 of 2026) added Kyrgyzstan and Vietnam and stated that the 240-hour visa-free transit then covered 57 countries. Eligible travellers need an ordinary passport and an interline ticket with a confirmed date to a third country or region. Under that visa-free transit, they may enter through 65 open ports, including ports in Beijing and Shanghai, and stay in the designated areas for no more than 240 hours. Shanghai's English page, updated on 20 August 2026 and citing the NIA, describes the same ports and the permitted areas across 24 provincial-level regions.
A ticket back to the country you departed from does not match the words "a third country or region". Whether a particular onward stop is accepted, including a stop in Hong Kong, Macao or Taiwan, is a question for the airline and the port before you buy. The inspection officer applies the rule in force that day.
The stay is short for medical care. Planning lead time is why a record, a date and a recovery do not fit every itinerary. A sedated endoscopy visit needs preparation, a companion and a report that may not be finished inside a transit window. Do not book a procedure that might need admission, or a result you must wait for, on the assumption that the permission can be stretched.
If a finding means you need to stay longer, contact the local exit-entry authority and follow its written instructions. A hospital letter can describe the medical situation. It is not an extension.
Hainan visa-free entry
The same 20 August 2026 announcement stated that Hainan's visa-free entry then covered 61 countries. Eligible nationals may enter through Hainan's open ports and stay within Hainan for no more than 30 days. The same activity limits apply: tourism, business, visits and family visits are named; work, study and news reporting are not.
The Boao Lecheng International Medical Tourism Pilot Zone is a place where some medicines and devices have a special regulatory path. Availability of any one of them depends on the current approval, the institution's authorisation and the treating clinician. Confirm that in writing with the provider. A zone on a map is not an appointment. A stay that is limited to Hainan is not permission to continue to another province. Ask the authority what document that onward journey needs.
A consular visa
If you need a visa, apply in the category that matches the purpose you will actually declare, using the instructions of the embassy or consulate that serves you. Do not pick a business category only because you want a checkup, and do not hide a material purpose. Supporting papers, if that office asks for them, should be accurate. An appointment confirmation can explain a plan. It does not decide eligibility.
Three inbound patient groups is the page on why a passport label does not choose a city or a bill. Direct billing and language support is a separate confirmation you still need after you have lawful entry.
At the hospital
Lawful entry and hospital registration are different desks. Bring the original passport and the proof of entry the hospital asks for. Ask, before you fly, how that hospital registers a foreign visitor, whether a deposit is required, which language the report will be in, and how you will pay. Staff at registration are not the authority on your visa category.
Where PriumCare fits in China
Where that support is confirmed in your service scope, PriumCare can help you prepare an appointment confirmation and a written estimate for your own visa file.
We do not diagnose, we do not treat, and we do not decide the visa category. We do not issue invitation letters and we do not submit or handle visa applications. PriumCare does not mark up hospital fees. For a sudden or worsening emergency, call 120.
Is there a medical-tourism visa for a checkup?
The NIA notices reviewed for this page do not describe one for routine outpatient care. Ask the embassy or consulate that serves you. Do not rely on a hospital booking as the category.
Does a return ticket qualify for the 240-hour visa-free transit?
The published rule asks for onward travel to a third country or region. A return to the country of departure does not match those words. Confirm the itinerary with the airline before you pay for it.
Does the 240-hour visa-free transit list medical care as a permitted activity?
No. The 20 August 2026 notice names tourism, business, visits and family visits. Work, study and news reporting need a visa. Ask the airline and the port whether your purpose is accepted.
What if treatment runs past the permission?
Contact the local exit-entry authority promptly and follow its written instructions. A hospital letter is not itself an approval to remain.
Can a coordinator issue the invitation?
No. A coordinator can give you an appointment confirmation and a written estimate. Only the embassy, consulate or immigration authority decides the documents and the entry.
Where do I recheck the numbers?
On the National Immigration Administration site, and with the embassy that serves you, before you buy tickets. The counts of 57 and 61 countries are taken from the announcement of 20 August 2026. They can change.
